Authorization and Acknowledgement

I look forward to sharing my time and talents with Yerkes Observatory. In serving as a volunteer, I recognize that my actions may reflect on the organization and, therefore, I promise to act in a professional, respectful fashion at all times. I understand that filling out an application does not guarantee acceptance into Yerkes Observatory’s Volunteer Program.

I understand that, if accepted into the Yerkes Observatory’s Volunteer Program, I am not an employee of Yerkes Observatory. Rather, I am a volunteer, and Yerkes Observatory can terminate my volunteer status at any time, with or without cause.

I also understand that, while volunteering at Yerkes Observatory, I may come into contact with sensitive information. I acknowledge that museum/client information is confidential and cannot be disclosed, shared or repeated.

By signing this agreement, I give Yerkes Observatory permission to contact my former employers, schools, military service and references, and to conduct a criminal background check. The information provided on this form is true and complete to the best of my knowledge. I certify that the statements on this form are true and agree that any falsifications or omissions by me in connection with this application or attachments may result in my dismissal from Yerkes Observatory’s Volunteer Program.
